If your stomach often rumbles and you pass a lot of gas, it can be very embarrassing and you may not know the cause. In Traditional Chinese Medicine, there are two causes.

The first is emotional issues, such as being easily angered, sulking, irritable and short-tempered. Some people vent their emotions, while others tend to keep them inside. Either way, over time this leads to stagnation of liver qi; liver qi stagnation transforms into fire, which affects the spleen and stomach and gradually causes stomach heat. Symptoms may include discomfort in the epigastrium, bad breath, abdominal bloating, nausea, and vomiting.

The second stomach (spleen and stomach in TCM) is cold, so one is always sensitive to cold; especially now that the weather is cool, even a slight breeze can cause abdominal pain and diarrhea. Also, at night when it is cooler and yin is excessive, coupled with inherent spleen and stomach deficiency and kidney yang insufficiency, the qi in the abdomen descends and moves up and down in the mid-abdomen, producing borborygmi. In this situation, formulas that regulate and promote qi, strengthen the spleen and eliminate dampness, and clear heat and promote urination can be very effective.
